WellSky describes Personal Care, formerly ClearCare, as "purpose-built to support the unique needs of non-medical home care organizations." Its scheduling page describes AI caregiver matching that weighs driving distance, preferred weekly hours, skills, overtime and schedule history. Schedulers can send open shifts to selected caregivers by text, email or the app, and get alerts when a caregiver is running late or nearing overtime (AxisCare scheduling).

How Do They Handle EVV?

All three capture the six data points federal law requires. Federal law sets six EVV elements: service type, the person receiving it, date, location, caregiver, and start and end times (42 U.S.C. 1396b, Cornell LII). States had to require it for Medicaid personal care visits from January 1, 2020.

AxisCare captures EVV through its mobile app or telephony, with GPS at clock-in and clock-out. It names HHAeXchange, Sandata, AuthentiCare, CareBridge and Netsmart as aggregators it works with. WellSky and AlayaCare win on payroll because both describe it as built in. WellSky calls Personal Care "the all-in-one solution for running your business, from scheduling to billing and payroll." It lists Change Healthcare for Medicaid and insurance claims and Worldpay for card payments (WellSky partner integrations). AlayaCare's homepage says it puts "intake, scheduling, care management, billing, payroll" into one system (AlayaCare).

AxisCare is strong on billing, especially for mixed payers. It syncs with QuickBooks for private pay and supports Medicaid, VA and long-term care insurance billing, with a family payment portal. For payroll, it says it connects with "many different CRM and payroll tools" rather than running payroll itself (AxisCare billing).

The app is free for customers, and caregivers can view and accept open shifts or swap shifts without calling the office (AxisCare caregiver app). Those are AxisCare's own figures.

WellSky's app is built for caregivers clocking in for a shift and for administrators updating schedules (WellSky Personal Care overview, January 2025). Its Family Room portal gives families the care calendar and shift notes, and lets them pay or split a bill (WellSky Personal Care). WellSky's Connect API is also publicly documented, with 22 subscription events. It has an applicant status event but no shift event, and only a clock-out event. Open shifts and late clock-ins have to be checked by polling (WellSky Connect API). AlayaCare streams 25 event types, including visit-clock-in, through Amazon SQS queues rather than webhooks.
